Specifics of a treatment plan are agreed individually after diagnostics. The form is for an initial exchange - please describe your situation in plain words.

Please do not send full medical documentation before I ask for it. If your case requires detailed assessment, I may ask for additional information or diagnostic images.

The form is not for acute medical issues. In an emergency please contact the nearest dental emergency service.
